I. Who We Are
This Notice of Privacy Practices describes how TGHCO Inc, operating as Mars by GHC, and its affiliates and medical partner network (collectively, "we" or "us") may use and disclose your Protected Health Information ("PHI") and how you can access this information. This notice applies to all records of your care generated or maintained by our organization.
II. Our Privacy Obligations
We are required by law to:
- Maintain the privacy and confidentiality of your Protected Health Information
- Provide you with this notice of our legal duties and privacy practices
- Abide by the terms of this notice currently in effect
- Notify you in the event of a breach of your unsecured PHI
III. Permissible Uses and Disclosures Without Written Authorization
We may use or disclose your PHI without your written authorization in the following circumstances:
- Treatment: To provide, coordinate, or manage your healthcare and related services, including consultations between providers.
- Payment: To obtain payment for services, including billing, claims management, and collection activities.
- Healthcare Operations: For activities such as quality assessment, staff training, compliance programs, and business planning.
- Public Health Activities: As required for disease reporting, vital statistics, or public health surveillance.
- Health Oversight Activities: To health oversight agencies for audits, investigations, or inspections authorized by law.
- Judicial and Administrative Proceedings: In response to a court order, subpoena, or other lawful process.
- Law Enforcement: To law enforcement officials as required by law or in response to valid legal process.
- To Avert a Serious Threat: When necessary to prevent or lessen a serious and imminent threat to health or safety.
- Workers' Compensation: As authorized by workers' compensation laws.
- Research: For research purposes when approved by an Institutional Review Board.
- As Required by Law: When disclosure is mandated by federal, state, or local law.
IV. Uses and Disclosures Requiring Written Authorization
We will obtain your written authorization before using or disclosing your PHI for purposes not described above, including:
- Marketing communications
- Sale of your PHI
- Most uses of psychotherapy notes (if applicable)
- Any other purpose not listed in Section III
You may revoke any written authorization at any time by sending a written request to support@marsghc.com. Revocation will not affect any uses or disclosures already made in reliance on your prior authorization.
V. Your Rights Regarding Protected Health Information
You have the following rights regarding your PHI:
- Right to Request Restrictions: You may request restrictions on certain uses and disclosures of your PHI. We are not required to agree to all requests but will comply where legally mandated.
- Right to Confidential Communications: You may request that we communicate with you by alternative means or at alternative locations.
- Right to Inspect and Copy: You may request to inspect and obtain copies of your PHI maintained by us.
- Right to Request Amendments: You may request amendments to your PHI if you believe the information is incorrect or incomplete.
- Right to an Accounting of Disclosures: You may request a list of instances where we have disclosed your PHI for purposes other than treatment, payment, or healthcare operations.
- Right to a Paper Copy: You have the right to obtain a paper copy of this notice upon request.
VI. Effective Date and Duration
This Notice of Privacy Practices is effective as of January 1, 2026. We reserve the right to change the terms of this notice and to make new provisions effective for all PHI we maintain. Revised notices will be posted on our platform.
