By using Mars by GHC services, you acknowledge and agree that:

- You authorize your healthcare provider to deliver care for weight management via telehealth

- You may withdraw consent at any time without affecting your right to future care
- You understand that your provider may determine that telehealth is not appropriate for your condition, in which case you will be referred to in-person care
3. HIPAA Consent
In accordance with the Health Insurance Portability and Accountability Act (HIPAA), effective April 14, 2003, you acknowledge the following:
- Your health information will be kept confidential and used only as described in our Privacy Practices notice
- Electronic health records are stored securely with appropriate access controls
- We may contact you regarding appointment reminders, treatment updates, or care coordination
- Third-party vendors who access your data are bound by business associate agreements
- You have the right to request access to, correction of, or restrictions on the use of your health information
4. Financial Consent
By enrolling in Mars by GHC services, you authorize TGHCO Inc to:
- Securely store your payment information on file
- Process subscription charges on your scheduled billing date
- Automatically renew your subscription unless you cancel in accordance with our Cancellation Policy
- Process charges for additional services you elect to receive
5. Shipping Authorization
You acknowledge and agree that:
- Prescribed medications are considered dispensed upon shipment from the pharmacy, not upon delivery
- TGHCO Inc is not liable for shipping delays, carrier errors, or lost packages once medications have been transferred to the shipping carrier
- You are responsible for providing an accurate shipping address and ensuring someone is available to receive temperature-sensitive medications if applicable
6. Important Disclosures
- Certain medications prescribed through our platform may be compounded and are prepared by licensed compounding pharmacies
- Your provider will determine the most appropriate treatment based on your health profile
- Results vary by individual and are not guaranteed
- Some content on our platform may be generated or assisted by artificial intelligence tools
